This form is for charities, community organisations, support workers, social prescribers and other professionals who would like to refer someone for a fully funded confidence portrait session.

A referral may be suitable when someone would benefit from the experience but cannot currently afford either the standard £225 price or the supported £125 price.

The person you refer will receive exactly the same session as every other client. This includes preparation beforehand, patient guidance, professional lighting, a private gallery and five carefully edited photographs. The only difference is that the full cost is covered for them.

You do not need to provide financial evidence or share sensitive personal information. Please give only the details needed to explain why the session may be helpful. The person must know about the referral and agree to being contacted before a session can be arranged.

Fully funded sessions are limited. Completing this form does not guarantee an immediate place. I will review the referral personally and contact you to discuss availability and the next steps.
